These are not hypothetical risks. These are patterns that appear in healthcare organizations consistently, across every size and specialty. They are fixable. But they are much harder to fix after deployment than before.
1. Deploying AI in clinical workflows before governance is in place
A department head finds an AI tool that could save hours of documentation time. They start using it. Other staff see the results and start using it too. Within a month, twelve people are entering clinical notes into a tool that nobody in compliance has reviewed.
The tool does not have a BAA. The tool's terms of service allow the vendor to use input data for model training. Patient names, diagnoses, and treatment plans are now in a system that the health system does not control.
This is not a privacy concern. It is a HIPAA violation in progress. The cost of discovering this proactively: a compliance review and a policy update. The cost of discovering this after an incident: OCR investigation, potential fines, mandatory breach notification, and reputational damage.
2. Buying AI tools from vendors who do not understand HIPAA
The vendor demo looks great. The tool is fast, accurate, and easy to use. The sales team says "yes, we are HIPAA compliant." The procurement team moves forward.
Then the BAA negotiation begins. The vendor's standard BAA does not cover AI-processed data specifically. Their data retention policy is not configurable. Their audit logging does not meet the specificity requirements the compliance team needs. The vendor's security team is responsive but cannot answer questions about how PHI is handled within the AI model.
The procurement process stalls. The department that was excited about the tool has been using it for three months already, because the pilot started before procurement finished.
3. Training only the IT team
IT receives the training. IT understands the tool. IT configures the system. IT rolls it out to clinical staff with a one-page instruction sheet and a 20-minute lunch-and-learn.
Clinical staff encounter the tool in their workflow. It does not work the way they expect. The output format does not match their documentation style. The suggested responses do not align with their clinical judgment. They work around the tool instead of using it. Some abandon it entirely. Others use it but override its suggestions without understanding what the tool was trying to do.
The investment in the AI tool is wasted, not because the tool was bad, but because the people who needed to use it were not trained to use it.
4. Treating AI implementation as a project with an end date
The project plan says: pilot in Q1, evaluate in Q2, roll out in Q3, done by Q4. The executive sponsor moves on to the next initiative.
AI implementation is not a project. It is a capability the organization develops over time. Models update. Workflows change. Staff turn over. The governance document needs quarterly review. Training needs to be repeated for new hires. The tools need to be maintained and updated.
Organizations that treat AI as a project end up with tools that degrade over time. The model changed and nobody noticed the output quality dropped. A staff member left and the person who understood the tool's configuration was not replaced. The governance document became outdated because nobody owned the update cycle.
5. Skipping the data quality step
The AI tool is deployed on clinical data that has been accumulating for years. Provider names are formatted inconsistently. Diagnosis codes have been entered using three different coding systems. Medication lists contain free-text entries that vary by clinic location.
The AI does not fix this. It processes it. The outputs look polished and professional. They are also built on data that contains duplicates, inconsistencies, and gaps. The AI's analysis of patient trends reflects the data quality, not the clinical reality.
Cleaning the data before AI deployment is not a technology project. It is a data hygiene step that most health systems skip because it is boring and time-consuming. The cost of skipping it: AI outputs that look right but are not, and a false confidence that spreads across the organization.
