The pharma industry has spent decades building compliance infrastructure. Validation protocols, audit trails, change control documentation, 21 CFR Part 11 compliance. That infrastructure exists because mistakes in pharmaceutical environments are not just expensive. They are dangerous.
And yet, when it comes to AI deployment, most pharma companies are making the same three mistakes they would never tolerate in any other regulated process.
Mistake 1: Deploying without validation documentation
In pharmaceutical manufacturing, you would never put a new piece of equipment into production without an Installation Qualification, Operational Qualification, and Performance Qualification. The documentation exists before the equipment runs.
AI tools are being deployed in pharma environments without any of that. Teams adopt Claude, ChatGPT, or custom AI workflows for data analysis, literature review, or report generation. No validation protocol. No documented testing against known outputs. No evidence that the tool performs consistently under the conditions it will be used in.
This is a 483 observation waiting to happen. When an FDA inspector asks how an AI tool was validated before it was used in a regulated workflow, "we tried it and it seemed to work" is not an acceptable answer. It would not be acceptable for a centrifuge. It is not acceptable for an AI model.
Mistake 2: Treating AI outputs as final without human review
GxP requirements mandate human oversight for critical decisions. Most AI vendors do not design their tools with this in mind. The tool generates an output. The user trusts the output. The output enters a regulated workflow without a documented review step.
The problem is not that AI outputs are wrong. The problem is that when they are wrong, there is no process to catch it. A literature summary that omits a relevant study. A regulatory submission draft that mischaracterizes a data point. An audit trail entry that contains an AI-generated description that does not match the actual change.
Human review loops need to be designed into the workflow before the AI is deployed, not added after someone notices an error in a submission.
Mistake 3: Skipping data governance because "the AI handles it"
Pharmaceutical data is rarely clean. Clinical trial databases have inconsistent naming conventions. Manufacturing data spans multiple systems with different schemas. Billing and compliance data carries years of legacy formatting decisions.
AI tools do not fix bad data. They process it. If the input data is inconsistent, the output will reflect that inconsistency. If the data contains duplicates, the AI will incorporate duplicates into its analysis. If field definitions vary across systems, the AI will not reconcile them unless specifically instructed.
The companies that skip data governance before AI deployment discover this the hard way: the AI's outputs look polished and professional, but the underlying data problems are now embedded in the AI's work product. Cleaning this up after deployment is significantly more expensive than cleaning the data first.
The right sequence
Governance first. Define what the AI tool will do, what data it will access, what validation protocol it must pass, and who reviews its outputs.
Training second. The people who will use the AI tool, and the people who will review its outputs, need to understand how it works, what its limitations are, and what a correct vs incorrect output looks like.
Validated tooling third. Deploy the AI tool within the governance framework. Document the validation. Maintain the audit trail.
This sequence is not slower. It is faster than deploying first, discovering problems, pulling the tool back, building governance retroactively, retraining the team, and redeploying. That path takes months. The governance-first path takes weeks.
